Statement of Post-HS Activities

Please provide a complete listing of work, military, school or other pertinent activities covering the period you were not enrolled in an educational institution until your proposed entry date to UNI.  This should include approximate dates, type of activity, and city and state. 

If you graduated from high school more than five years ago, you need to list activities only for the five year period immediately preceding your proposed entry to the university.
