Satisfactory Academic Progress (SAP) Appeal

If your financial aid has been suspended due to not meeting Satisfactory Academic Progress (SAP), you may complete the SAP Appeal to request to have your aid reinstated. Appeals will be reviewed after all items have been received.

  • Fall appeals must be submitted by October 1st.
  • Spring appeals must be submitted by March 1st.
  • Late appeals will be considered on a case-by-case basis (no appeals will be accepted after the semester has ended).

This form must be completed by the student whose name appears on the form. A copy of this form will be sent to the UNI email address you enter below. 

The SAP Appeal committee meets once a week to review appeals. We will reach out if additional information is required. Once your appeal has been processed, you will receive an email notification with our decision.

Plan of Study

As part of the appeal, you must complete a Plan of Study with your academic advisor. Deciding or changing majors may meet with an advisor in the Office of Academic Advising. If you are within 30 credit hours of graduating, you may meet with a record analyst in the Office of the Registrar.

You may upload a signed Plan of Study using this form, you or your advisor may email the signed Plan of Study to jennifer.sullivan@uni.edu, or you may drop off the signed Plan of Study at the Office of Financial Aid & Scholarships.

Appeal Essay

If your essay includes the disclosure of discrimination, harassment, sexual violence, sexual harassment, or other sexual misconduct, please be aware that your appeal will be shared with the University Title IX Officer so that appropriate resources may be offered to you. For more information, please refer to the UNI Policy on Discrimination, Harassment and Sexual Misconduct.

Please submit any documentation you have to support what you describe in your essay. Please review our SAP website for examples of documentation.

If your documentation contains personally identifiable information (e.g., Social Security Numbers, birth dates, etc.), please do not upload to this form. You may drop off these documents at the Office of Financial Aid & Scholarships or fax documentation to our office at 319-273-2320.
