Acceptance Fee Deferral Request

Complete the Acceptance Fee Deferral form if payment of the acceptance fee presents a financial hardship.

Application Fee Waiver Request

Complete and return the Application Fee Waiver Request form if payment of the application fee presents a financial hardship.

Health Requirements (Immunization and Insurance) (PDF)

Proof of immunization form for new students (required).

International Student Fee Deferral Form

International students can choose to defer their application fee using the International Fee Deferral Form. Doing so means students are not required to pay the fee until they are enrolled at UNI and receive their first university bill.

Indian Hills/UNI Transfer Pathway Program Application

The Indian Hills/UNI Transfer Pathway Program Application is for international students interested in the Transfer Pathway Program allowing students to start at Indian Hills Community College and then continue at UNI to complete their bachelor's degree.

Phi Theta Kappa Scholarship Confirmation

Phi Theta Kappa Scholarship Confirmation is for new transfer students to the University of Northern Iowa who have earned 30 or more semester hours (after high school graduation) at the time of application, have maintained a 3.5 cumulative transfer grade point average, and hold membership in Phi Theta Kappa International Honor Society.

Post-Secondary Enrollment Option Act (PSEO) Notice of Student Registration Form (PDF)

Post-Secondary Enrollment Option Act (PSEO) Notice of Student Registration Form is for students to enroll and register for courses at the University of Northern Iowa while currently enrolled in high school. Must be accompanied by the Authorization for Dual Enrollment and application for undergraduate admission.
